You will assist in maintaining their independence and enhancing their quality of life by delivering person centre care in various settings such home environments, day centrres, work enviroment, social settings as well as in and around the community, Main Responsibilities: Support individuals in maintaining a clean and safe living environment. Assist with meal preparation, feeding, and dietary requirements as needed. Engage clients in meaningful activities to promote social interaction and mental stimulation. Collaborate with healthcare professionals, families, and other team members to ensure holistic care for each individual. Prompt individuals to take medication Support client to access the community including social interactions Provide support in building specific lifeskills Where required, provide admin/PA support Document and report any changes in clients'' health or behaviour.
