About the Role: As a Senior Residential Children s Practitioner, you will: Care and Documentation Management: Review and update care plans, risk assessments (RAMPs), and Positive Behaviour Plans to ensure they reflect the current needs of each young person. Oversee and update Pathfinders weekly to track progress. Ensure all health and medical events are documented in sufficient detail. Collaborate with the Registered Manager and social workers to maintain comprehensive records of each child s history, promoting their identity.
