Social Worker - Assessment and Care Team Newham Council Role Overview: As a Social Worker within the Adults Assessment and Care Team, the successful candidate will work collaboratively with other professionals to assess the needs of adults with a variety of complex care needs, including physical disabilities, learning disabilities, mental health issues, and older adults. The role involves carrying out assessments, creating care plans, and supporting service users through the provision of tailored interventions that promote independence and well-being. Key Responsibilities: Conduct comprehensive assessments of individuals'' social care needs. Develop, review, and implement care plans in line with policies and procedures. Person Specification: The ideal candidate will be a qualified, experienced social worker with: A Social Work degree (or equivalent) and SWE registration.
